The clinical profile of neurological manifestations in HIV patients has undergone a shift in recent years with opportunistic infections being controlled with combination anti retroviral therapy and the advent of drugs which have higher central nervous system penetrability. Aims and Objectives To study the clinical, investigation profile and various neurological disorders in HIV patients on anti-retroviral therapy. Materials and Methods Fifty HIV patients with neurological manifestations were studied. A complete neurological examination including neurocognitive functioning using Montreal Cognitive Assessment and HIV Dementia scale were assessed. Apart from relevant investigations, CD4 count, Computed Tomography/Magnetic Resonance Imaging brain, cerebrovascular fluid analysis was done where required. Results Neurocognitive disorders formed the largest group with 42% suffering from HIV associated Neurocognitive Disorders .Among them asymptomatic neurocognitive impairment was seen in 28%; mild neurocognitive disorder in 12%, and 2% had HIVà ¢Ã¢â€š ¬Ã‚ associated dementia. Opportunistic infections of the nervous system accounted for 32%, with meningitis being the most common. Four patients had space occupying lesions of central nervous system; four tuberculomas and one toxoplasmosis. Conclusion With the advent of highly active retroviral therapy HIV patients have longer life spans with suppression of viral load leading to decrease in opportunistic infections of the nervous system. MATERIALS AND METHODS The study was done on HIV patients getting admitted in a tertiary hospital and research center in Bijapur from April 2014 to June 2014.Fifty HIV positive patients with clinical neurological manifestations during this period have been included in the study. Inclusion criteria HIV patients on combined anti retroviral therapy having signs and symptoms caused by neurological dysfunction. Exclusion criteria Patients with pre-existing neurological conditions like epilepsy, mental retardation, cerebrovascular diseases, parkinsonism, movement disorders not attributable to the HIV. Patients not on any therapy. Patients who have discontinued their treatment for duration of more than a month. An informed consent was obtained from the patients prior to their inclusion in the study. A detailed history, general physical examination with thorough neurological examination was done. The patients were also assessed for their cognitive functions as a part of neurological examination using Montreal Cognitive Assessment (MoCA)2, International HIV Dementia Scale (IHDS)3.If cognition was affected it was quantified based on the effect it had on everyday activities using Lawton scale for Instrumental Activities of Daily Living4. The tests were performed on all patients in study. Patients were classified as HAND based on the scoring and clinical judgment of impaired performance in at least two domains of MoCA and IHDS tests. MoCA was assessed on delayed recall, executive function, visuospatial function, attention, language function, abstract thought and orientation. The maximal score was 30 and patients with IHDS was scored on three variables motor speed, psychomotor speed and memory recall, the patients are scored on each of these. The maximum possible score was 12 points. A patient with a score of The patients with HAND were further subdivided into Asymptomatic Neurocognitive Impairment (ANI), Mild Neurocognitive Disorder (MND) or HIVà ¢Ã¢â€š ¬Ã‚ associated dementia (HAD) using revised research criteria for HIV associated neurocognitive disorders (HAND)5.The patients in study group not diagnosed with HAND were used as reference group for standardized neuropsychological tests and compared. RESULTS The mean age of the patients was 32.3 years with all of them being between 20 to 45 years. There were 36 males and 14 females. The mean years of education of the group was 6.41 years with only 30% being employed in professional jobs and the rest either unemployed or intermittently worked as unskilled labor.(Table 1) The most common symptom in these patients was headache seen in 42% of cases followed by fever (30%) and altered sensorium(10%).Seizures were present in three of the five patients with with space occupying lesions. (Table 2) Neurocognitive dysfunction were the most common neurological disorders (42%) followed by opportunistic infections(32%). The MoCA and IHAD test results were used to diagnose the patients with HAND. These patient data were then compared with the other HIV patients who underwent these tests. ANI was present in 28% ,12% had MND and 2% were suffering from HAD. The patients with neurocognitive disorders were included in group A and the rest in group B. (Table 3) Cryptococcal meningitis was the most common infection accounting for 14% followed by tubercular meningitis with 12% of the cases. Co infection was seen in six cases. There was one case each of myelitis and toxoplasma.Space occupying lesions were found in five cases, four of which were tuberculomas and one lymphoma.(Table 4) According to WHO immunological classification for established HIV infection based on CD4 counts6 ;52% had severe disease, 24% had advanced disease , 4% had mild disease and remaining 4% did not have significant disease. Fifty six percent of patients in group A had CD4 count less than 200cells/mm3 [severe disease] and of the 21 patients in group B, 71.5% had severe disease.(Table 4) Nineteen patients underwent CT scan and/or MRI of which 13 had normal scan results ,five patients were diagnosed to be having intracranial masses,four tuberculomas and one lymphoma .Toxoplasma was suspected in one patient which was later confirmed with finding Toxoplasma- IgM antibodies in blood. One patient had features on CT suggestive of encephalomyelitis. CSF analysis was done in 17 patients of whom seven were diagnosed with cryptococcal meningitis by India ink preparations. Six patients had findings suggestive of tubercular meningitis. DISCUSSION Neurologic abnormalities are common in late stages of HIV infection and are an AIDS defining condition. Central nervous system disease occurs in 40-90% of HIV positive patients. The predominant cell types that are infected are monocytes and macrophages. Virus may enter the brain through infected monocytes and release cytokines that are toxic to neurons as well as chemotactic factors that lead to infiltration of the brain with inflammatory cells.7 Our study showed a male preponderance towards neurological manifestations in HIV with 72% of the patients being male similar results were seen in other studies. Deshpande et al reported 87.5% male patients8 and Chan LG et al had 93.3% male subjects9.This could be attributed to higher incidence of multiple sexual partners in males compared to females, except for high risk groups like commercial sex workers. In our study the most common manifestation of HIV patients with neurological disorder was headache, seen in 42% patients which corroborated with other studies. Sharma SK et al. also reported headache as the commonest neurological manifestation seen in 62.5% cases10. The neurological symptoms associated with HAND like forgetfulness, mental slowness, poor concentration, clumsiness, tremors, apathy were seen in less than six percent of people although HAND accounted for 42% cases. This data suggests the huge propensity to miss the diagnosis of cognitive dysfunction as the disease usually exists in its subclinical form with little or no symptoms. The few patients who did have these symptoms did not associate them to their disease. The approach towards neurological diseases in HIV has always been directed towards opportunistic infections. Most studies on neurological complications did not assess for neurocognitive disorders although some studies have reported on dementia. In our study the prevalence of neuro-cognitive disorders surpassed opportunistic infections of CNS in HIV patients. HAND was seen in 42% of the cases similar to study done by Chan LG et al. in Singapore where the prevalence was 22.7% .They also used the revised 2007 updated research nosology for HAND. Singh R et al. reported a 33.65% incidence of HIV associated Dementia.This study showed similar result to our study but the assessment was done using MMSE11. Deshpande AK et al reported only 6% patients with dementia.Satishchandra et al. reported dementia in only four patients out of 100 patients12. Sharma et al. did not report on neurocognitive disorders.All these studies did not employ an objective methodology for assessing the cognitive function and based their results on clinical and neurological examination findings. It is very common to assess cognitive impairment based on clinical judgement and brief bedside neurological examination, this methodology though novel severely limits the providers ability to diagnose subclinical forms of cognitive dysfunction and being subjective their results cannot be used to compare with studies were standardised tests were employed. The CD4 counts in patients with HAND are below 200 cells/mm3 in 71.4% of cases.Thus we concluded that HAND is usually seen with lower CD4 counts associated with severe form of disease. Chan LG et al. reported similar findings were HAND was seen in patients with CD4 counts less than 200cells/mm3 in 63.3% , Singh R et al. reported similar findings in 83.52% patients. Our study did have its shortcomings, our sample size was smaller and strict compartmentalization of neurological disorders into opportunistic infections and neurocognitive dysfunction may have given a lower incidence of opportunistic infections .Thus further studies need to be taken up with larger sample sizes using standardized cognitive assessment tests . With the widespread use of HAART the incidence and mortality associated with HIV is decreasing throughout the world.By keeping the of viral loads suppressed and prolonging the lifespan of HIV individuals a new profile of neurological disorders is emerging which warrants a change in approach to the management.Along with opportunistic infections emphasis must be made towards diseases caused by direct effect of the virus on the nervous system.Neurocognitive and neuropsychiatric illnesses should be thoroughly investigated using standardised mental status examination (those acceptable in updated research nosology for HAND considering resource limited contexts) 5; as they can be subclinically present severely affecting the patient’s daily activities and add to the overall burden of the disease.

Loose Ends Still Untied Aurora is not known to be the greatest town in the suburbs of Chicago, so it is a typical move for the people from my side of town to claim residence in Naperville. I will be the first to admit that I have often betrayed my hometown and laid claim to its relatively glamorous neighbor. Naperville is one of the country’s â€Å"best places to raise a family,† or so I have heard. I wouldn’t be too surprised, considering the amount of wealth that flows through the town. Naperville offers a mix of people, professionals and their families of various ethnicities and backgrounds; however, it lacks true culture diversity. Even though there are whites, blacks, Hispanics, Chinese, Japanese, Indians, etc., few of its youths are conscious of the various backgrounds because of the economic equality of everyone: everyone is equally rich in Naperville (a point of which I and my fellow Aurorans regularly accused our Naperville schoolmates). My high school consisted of a decent racial blend, and despite a few cultural cliques, everyone was White in thought and in wallet. I did not hold this view at the time, but I had yet to be exposed to reality then. When I came to the University of Illinois, I was accompanied by a significant force of my high school peers, including all but two of my closest friends. During the first few weeks of school, when everybody was meeting everybody else, I was busy hanging out with my standard high school group and, thus, missed much of the opportunity to make a bounty of new friends. I did, however, meet one person who has become my closest friend and who sparked my introduction to reality. I went to visit him over spring break. It was a Friday, a little past noon. Nick Hornby’s novel, ‘About a boy’ is a story about two boys. A 12 year old, named Marcus, who has a suicidal mother. And a 36 year old child-like-man called Will Freeman. Will, has never worked in his life, he was been living of the royalties of a song that his father had wrote for Christmas, over 70 years ago. Marcus is very much a ‘mummy’s boy.’ He is so close to his mother that his taste in music and clothes are identical. When he starts his new school, he realises that he is an easy target for bullies as his hair is cut ‘dodgy’ by his mum and he has a tendency to sing aloud in class†¦. When we see Marcus in chapter one, we see a naive teenager. On the very first page of the book, Marcus’ mum Fiona suffers from a break up. â€Å"Have you spilt up now?† This very quote shows the naivete of Marcus. This childish questions, suggest that Marcus has not developed any level of maturity and has no means of comforting his mother. However, with this in mind, Marcus has a very logical way of thinking, and there are there are two valid reasons for this bold statement. When we do see the break up incident, Marcus realises that when people begin to do things together (almost like a family) they should gel and stay together. â€Å"He’d once shared a toilet with Roger, when they were both busting for a pee after a car journey. You’d think that if you’d peed with someone you ought to keep in touch with them somehow.† This logical thinking by Marcus suggest that he had thought about future with his mum and Roger, and that Roger would stay because of the family relationship that they all shared. The second reason why I believe he has a logical process is the pizza incident happens. When the break up argument began, they had just orders three pizzas. As we know Marcus and Fiona are ‘vegetarians’, but Roger wasn’t, so one of the pizza contained peperoni. â€Å"We’ll throw it away then?† Fiona suggests, but Marcus being Marcus, he promotes that they should â€Å"Pick the peperoni off.† This suggests that instead of throwing away a perfectly good pizza, Marcus thinks that they can still eat the pizza without the peperoni topping. Although Marcus is still very naive, we see a very different side of Marcus on the very past page of chapter 1. He is very protective of his mum. When they have fished eating and arguing, Marcus finds the TV remote and â€Å"Zapped through the channels. He didn’t want to watch any of the soaps, because soaps were full of trouble, and he was worried that the trouble in the soaps would remind his mum of the trouble she had in her own life.† This quote suggests that because of his mother’s break up’s and troubles, he did not want the TV reminding her of anything, which really shows how protective he is over his mother. Marcus has just moved house, which unfortunately means he starts a new school. In the first few chapters, we see life for Marcus in school, and it wasn’t going to be easy. He is different from his peers. There times in the book, which really shows how apart he is from everyone. Firstly, he arrives for school. EARLY. Not a typical act from a teenager. â€Å"He got to school early, went to the form room, sat down at his desk.† This is one of vey first examples, of why he is very different. Not many teenagers would willingly want to come to school early. The way he dresses, and the way he looks is another issue. â€Å"He was usually wearing the wrong shoes or the wrong trousers, and his haircut was wrong all the time.† This would mean that he would be an easy target for bullies. However, worst is still to come. â€Å"A song had slipped out yesterday during English.† When he had an English lesson, Marcus unexpectedly sang a song whilst the teacher was reading. As a kid, this really ‘sealed the deal’ for Marcus. Luckily, Marcus had found a few friends who were also outsiders like him. They were Nicky and Mark. Marcus meet them by an after school computer club. However, their relationship with each other was very timid. â€Å"He was being left out deliberately† Marcus was waiting his turn on the Gameboy, at this point in the book. Although he hasn’t done anything wrong, he was still singled out. Suddenly, things to a turn for the worse for their relationship. At lunchtime, a few bullies, who had been tormenting Marcus before, came up to Marcus and his friends, and began insulting them. When they had gone, Mark spoke out and said. â€Å"Marcus, we don’t want you hanging around with us anymore.† This shocked Marcus. He knew that if they were to hang out with him, they would all get bullied. These so called ‘friends’ that Marcus had, had been unsupportive and nothing like Marcus. This shows how different Marcus really is; he cannot fit in with people who are so similar to him. As have mentioned before, Marcus is overprotective over his mum, because of their closeness and the strong relationship they have. In the fifth chapter, we see Marcus worrying about his mother. This is caused by the constant crying habit his mom has developed. This had frightened him. â€Å"One Monday morning his mother started crying before breakfast, and it frightened him.† In this scenario in the book, Marcus understands that his mum is going through a tough time but he does not know what to say or do. â€Å"He didn’t know what to say.† So instead, when we went for his weekly shop, he stuck to the ‘economy sized packets of cereal, because he knew that is he had brought banded name, then his mum would be angry. This again shows how protective he is over his mum. With this in mind, he had realised that there was â€Å"an advantage of her terrible depression.† Without her knowing he bought the branded ‘Coco-Pops.† This shows that he was still taking advantage out of his mother’s sadness. In the final few page of chapter 7 we see an intense moment between Marcus and Fiona. Marcus finally confronts Fiona about her constant depression and the fact the she hasn’t been looking after him. â€Å"All you do is make my meals and I could do that. The rest of the time you just cry. That’s†¦ that’s no good. That’s no good to me.† This shows that Fiona does not know how to look after a child. However, Marcus does not help or resolve the matter; instead he heads off to his room. â€Å"He went upstairs to his room and played NBA Basketball with the earphones on.† This shows a level of immaturity and a certain lack of social skill. Fiona claims that â€Å"we’re not doing each other any good† to Marcus. This again is more evidence to prove that Fiona simply cannot look after Marcus. When we see Will in chapter eight, he describes Marcus as being â€Å"the weird kid.† Will thinks this about Marcus because of the way he acts. For example, when they were at the back of a car together, Marcus began ‘humming tunelessly’. This would not be a normal act performed by a teenager, and this led Will into believing he is strange. There is more evidence for Will to believe Marcus is weird. â€Å"‘You know Michael Jackson, right? He makes a million pounds a minute,’ said the weird kid.† When Marcus said this, Will thought he was making things up, which again led Will to believing he is weird. At the park, Marcus accidently killed a duck in the lake. In this part of the book, Marcus changes his mind about Will in a matter of minutes, from despising him, and then realising that he was one his side. â€Å"Who did this Will think he was?† Will was trying to be sarcastic about the matter, but Marcus, did not find it funny. Then, all of a sudden â€Å"Marcus looked up at him; maybe he didn’t hate him after all.† Will then made the incident look like it had affected them in front of the park-keeper. Marcus had changed his view about him, in a flash, without him doing anything. All of this suggests that Marcus has mixed opinions on will, on moment he is the hero, and the next he is just a stranger, In chapter nine, we sadly see a suicide attempt from Fiona. The events leading up to this moment, is very intense. On the day of the suicide attempt, Marcus is at Regent Park for a picnic with Suzie and Will. Whilst Will and Suzie were talking, Marcus was feeding the ducks, with loaf hard bread that Fiona had made, he had then accidently thrown the whole loaf in to the lake and a killed a duck. Hence the ‘Dead duck day.’ All of a sudden Marcus could see Fiona across the lake waving and smiling at him, he turned around to tell Suzie but when he looked back she was gone. â€Å"It was then that Marcus saw-or thought he saw-his mum. She was standing in front of them, blocking the path, and she was smiling. He waved and turned around to tell Suzie that she’d turned up, but when he looked back his mum wasn’t there.† This part of the book is based on symbolism. I think it represents symbolism because there is a link between what happened with the Dead duck and the fact that she was there, meant that it was a way of saying bye to Marcus. It was almost like a premonition for Marcus, that something was about to happen. â€Å"A new part of his life began, bang, without any warning at all.† When I saw this part in the book, it really surprised me. I wasn’t aware of how Fiona was so depressed, that caused her to a terrible act like this. As a reader, I would say that Fiona’s depressions was only recognised as just her crying most the time, I was not sure she would have gone to that greater length, and commit suicide. It is obvious that Marcus was hit hard by the fact that his mother had committed suicide. At the end of chapter nine, it reads â€Å"he knew the moment he walked in that it was something he’d have to think about forever.† At this point in the book, we see Marcus’ character really open out. This continues as we read on. When they reach the hospital, Marcus is in the waiting room, reflecting on other people troubles and compares this to his mum. â€Å"My mum’s not like these people. Supposing they think she is, though?† he believes that the hospital might treat her as being just a normal druggy.

I was also thinking whether I was doing this thing in the best possible time, in order to live the best life I could. The answer was that taking a shower was the best possible thing, which I could do in the early morning; however, the way I was doing it was not the best, because I was wasting to much clean water. Thus, in the future I will use less water during the shower, in order to save this water for other people. When I decided to go for a walk, I asked myself, if that ws the best thing I could do at that moment. The answer was no, because I had not done my homework yet, so I decided to stay home and study. I asked myself, if I was studying in the best possible way, the answer was no, because the TV was on. I turned off the TV, and continued to study more effectively. I asked myself, if I was doing it in the right time, the answer was no, because I should have started studying earlier. Till the end of the day I studied, and tried to do it in the best way. For example, my friend got pregnant, and was trying to hide this information from her mother. I always try to learn from my mistakes, and from mistakes of people, who surround me. I always try to take something for my personal growth from the ideas of other people. If I think that the way the person acts is right, then I try to follow this way. If I think that thhe way the person behaves is wrong, then I try to show this person what mistakes I think he/she makes, and to avoid these mistakes in my own life. For example, one of my friends thinks that good life is to have a lot of money, and to date celebrities. I do not agree with her, so I try to convince her that money and fame are not that important. I agree with Plato that happiness is a crucial element in achieving a good life (Russell 105). I also agree that everyone can achieve the state of happiness (Riel, 57). In order to do this, the person must work on ones personality. During these 24 hours, I noticed several faults in the way I live. For example, I was wasting too much time by being lazy. I was also wasting food, because I was eating a bit too much. Thus, I decided, that in the future I will be more active, will eat less, and will constantly ask myself, what thing of every possible thing, which I could be doing, I should do at the moment in order to make my life the best I can, and to make myself the happiest person I can be. To conclude, after taking into considerations the lessons which I learned, I changed the way I see good life. My new goal is to always be active, feel happy, and to have everything I need.
